Plan Of Care
Once you or your loved one has been admitted to Next Generation Home Care as our client, our Registered Nurse will schedule an appointment with the client and family members to make sure our customized care plan best fits your needs. We are here for you and to help you in any way possible. At this time, our nurse will gather personal and medical information and the client will assist the nurse in helping create the weekly Plan of Care such as: planning meals, bathing, laundry, change of linens, and transportation. This information is kept in a binder and placed in the client’s home.
Daily Care Notes
Our Next Generation caregivers keep detailed notes of their visits with you or your loved one for family caregivers and medical emergencies. We also have a daily log sheet that shows the duties and activities that are done with your caregiver to make sure excellent care is provided.
Keeping track of prescriptions is also a challenge for many people. Medication reminders given by a Next Generation caregiver can prevent our clients from forgetting to take their medication or overdosing, both of which leads to more serious health concerns. Each day, the caregiver will look at the care plan to determine her duties for the day.
In the event there is a sudden change in your loved ones health condition, we will alert the emergency contact immediately.
